From 2000 to 2015 we covered about 15 cases that we thought, after careful surgical cleaning, of reconstructing the missing skeletal part with Platelet Gel, autologous and bank bone, BMP and mesenchymal cells taken from the iliac crest. The main rule was the safety the debridement of the septic part. The advantages of this method, on substance losses of long bones, are: A) the bone consolidation time that is between 4 and 8 months from application. B) In comparison with the method of “Ilizarov” the least pain and non-flexion of the knee and ankle that is usually created during the descent of the fragment and the significantly shorter times. C) the possibility in association with the autologous graft to add donor-approved cips. D) compared with the “Masquelet” method the non-use of such a large amount of autologous bone cips, although recently associated with homologous bone cips. The disadvantages are: A) the higher costs. B) the availability of the transfusion center for patient withdrawals for the construction of the autologous platelet gel and a tissue bank C) It is not clear when the external fixer is removed (particularly delicate moment affecting all bone reconstruction methods).
