Mesenchymal stem cells (MSC) were described initially by Thomas Wharton. In the human body, MSC are located mainly in bone marrow, adipose tissue, muscle tissue, Wharton jelly (WJ), cord blood and lungs. It has been demonstrated that MSC do not induce immune response. It has been also proved that MSC, in vitro, have immunomodulatory effects on antigen presenting cells (APC), Natural Killer cells (NK) as well as T and B lymphocytes. MSC obtained from the WJ compared to MSC derived from other sources, have a higher proliferation potential. The reduced activity of pro-inflammatory cytokines, including interferon gamma (IFN-γ) and tumor necrosis factor alpha (TNF-α) have been observed, in the case of MSC derived from WJ, more significant. There are many clinical trials worldwide in neurological diseases based on the MSC.

The aim of the presentation is to present the Polish results of the use of MSC in children with neurological diseases: autism, cerebral palsy, and meningomyelocele. Typical protocol contains: qualifying visit, 5 injections  – i.v. or i.th.administration every 6-8 weeks, before each administration, assessment of the physiotherapist and neurologist. Assessment scales used by physiotherapists from 2017, earlier Likert scale (quality of life) are: GMFCS, GMFM, 6-minute walk test, Up and Go test and CGI scale. The first results show positive effects of MSC in selected neurological disorders in children. Administration of the WJ MSC is an option for patients who do not have their own umbilical cord blood. We need for more clinical trials using stem cells in the treatment of neurologic diseases in children.
